The 504 contribution is a percentage of "the Project cost, excluding administrative costs" (13 CFR 120.910). Enter that figure, not the loan amount.

How long has the business been generating revenue from its intended operations?

On a 504 project a New Business is one "in operation for 2 years or less at the time the loan is approved". A change of ownership can also be treated as a New Business, and the SOP has a carve-out for some expansions under identical ownership. Ask your CDC.
